ABSTRACT

【Objective】 To explore the epidemiological features and relational factors of accidental death among children under 5 years of age in rural area of Shaanxi Province. 【Methods】 A case-control study was conducted in the research, and children under 5 years old in nine National surveillance counties of Shaanxi were collected. The questionnaire of national survey of accidental injuries among children under 5 years of age was used to investigate the basic information of children, socio-demographic characteristics, child care status, injury occurrence condition. The mean, standard deviation and percentage were used to describe the basic situation and main characteristics of accidental death. The Chi-square test and Logistic regression methods were performed to explore the relational factors of accidental death of children. 【Results】 Of the 25 cases of accidental death of children under the age of five years old, 5 were traffic accidents (20.0%), 9 cases were falling (36.0%), and 11 were suffocation (44.0%). Age distribution showed that children of accidental suffocation were younger, with 90.9% (10 cases) of them under the age of 1 years old. Gender distribution showed that traffic accident deaths occurred to boys. Area distribution showed that falling and suffocation death mainly happened in Hanzhong, while traffic accidents death mainly in Weinan. When the accident happened, 8 caregivers were not on the scene. What was worse, among 17 caregivers who were on the scene of accident, only 4 kept an eye on children. Compared with 25 children in control group, 16 in case group had received health examination, and the difference showed statistical significance (χ2=8.672, P=0.003). Meanwhile, 9 main caregivers were mothers in the case group, compared with 14 in the control group. The Logistic regression analysis showed that compared with fathers, mothers as the children’ main caregivers could positively reduce accidental death of children (OR=0.016, 95% CI: 0.000 3-0.997, P=0.049). 【Conclusion】 To decrease the incidence rate and mortality of accidental death of children under the age of five years old, parenting behavior guidance, health examination, and targeted health education should be taken in Maternal and Child Health Care System as a routine work.

ABSTRACT

La Cannabis sativa es una planta que contiene componentes psicoactivos (principalmente tetrahidrocannabinol) y actualmente corresponde a la droga ilícita más consumida a nivel mundial. Además, desde el área de la salud mental, ha habido un creciente interés en evaluar la relación entre el consumo de marihuana y el desarrollo de trastornos mentales. En este contexto, considerando tanto este creciente aumento en su consumo a nivel mundial y el interés por conocer si está involucrada en la patogénesis de patologías de la esfera psiquiátrica, es clave analizar qué posibles riesgos de desarrollar patologías mentales presentan aquellos niños expuestos al tetrahidrocannabinol durante la gestación. A partir de esta situación, el objetivo de este FRISBEEs es determinar si los niños/as expuestos a THC durante su gestación tienen un mayor riesgo de patologías mentales, en comparación a aquellos niños no expuestos durante su gestación. Los materiales y métodos utilizados para responder esta pregunta fueron obtenidos a partir de una búsqueda bibliográfica en dos bases de datos, donde se analizó la evidencia disponible, y se seleccionó el estudio primario titulado "Maternal tobacco, cannabis and alcohol use during pregnancy and risk of adolescent psychotic symptoms in offspring", ya que era el que más se aproximaba a poder responder nuestra pregunta clínica. Este se analizó de forma crítica, llegando al resultado de que el estudio no fue concluyente en establecer una asociación entre el uso de cannabis y síntomas psicóticos. Como conclusión, dado que no se pudo llegar a establecer una asociación entre el uso de cannabis y el desarrollo de patologías mentales, se debería realizar más investigación sobre el tema dado la magnitud del consumo de cannabis a nivel mundial, para así poder llegar a conclusiones clínicas basadas en la evidencia y poder dar recomendaciones clínicas a las pacientes embarazadas.


Cannabis sativa is a plant that contains psychoactive components (mainly tetrahydrocannabinol) and currently corresponds to the most widely consumed illicit drug worldwide. In addition, from the area of mental health, there has been a growing interest in evaluating the relationship between marijuana use and the development of mental disorders. In this context, considering both this growing increase in its consumption worldwide and the interest in knowing if it is involved in the pathogenesis of pathologies in the psychiatric sphere, it is essential to analyze what possible risks of developing mental pathologies present those children exposed to tetrahydrocannabinol during gestation. Based on this situation, the objective of this FRISBEEs is to determine whether children exposed to THC during their pregnancy have a greater risk of mental pathologies, compared to those children not exposed during their pregnancy. The materials and methods used to answer this question were obtained from a bibliographic search in two databases, where the available evidence was analyzed, and the primary study entitled "Maternal tobacco, cannabis and alcohol use during pregnancy and risk of adolescent psychotic symptoms in offspring ", as he was the closest to answering our clinical question. This was critically analyzed, reaching the result that the study was not conclusive in establishing an association between the use of cannabis and psychotic symptoms. In conclusion, given that it was not possible to establish an association between the use of cannabis and the development of mental pathologies, more research should be carried out on the subject given the magnitude of cannabis use worldwide, in order to reach conclusions. evidence-based clinics and to be able to give clinical recommendations to pregnant patients

ABSTRACT

Resumen El confinamiento al igual que el sepultamiento son parte de las sofocaciones por carencia de aire respirable. Su etiología principal es la accidental, como ocurre con los mineros o los niños que juegan con bolsas de plástico o quedan atrapados dentro de lugares sin ventilación. En la actualidad se ha visto el aumento a nivel mundial sobre todo en países desarrollados, del uso de una bolsa plástica en conjunto con algún gas noble (principalmente helio) como método suicida "per se" o como proceso final de eutanasia. En el presente artículo se hará revisión de dos casos en Costa Rica, con sus hallazgos en la necropsia, la carencia de hallazgos toxicológicos y la importancia del escenario de muerte para poder emitir un criterio desde el punto de vista médico legal.


Abstract The confinement as well as the burial are part of the suffocations due to lack of breathable air. Its main etiology is accidental, as it happens with miners or the children who play with plastic bags or get trapped inside unventilated places. At present, the increase in the world has been seen especially in developed countries of the use of a plastic bag in conjunction with some noble gas (mainly helium) as a suicidal method or as a final process of euthanasia.

ABSTRACT

The oxygen consumption, oxygen consumption rate and suffocation point of different quality Whitmania pigra and Bellamya purificata were determined by hydrostatic breathing room method. The effects of feeding modes on growth of W.pigra were determined by biomass. The results showed that the oxygen consumption correlated positively with the weight of W.pigra and B. purificata(P<0.05), suffocation point increased with the increases of the weight(P<0.05).Oxygen consumption correlated negatively with the weight of W. pigra, the oxygen consumption rate of B.purificata first increased and then decreased with the increasing of the weight. Feeding modes had no significant effects on the finial weight, SGR, WGR, death rates of W. pigra. Feeding modes had significant effects on eating ratio. It suggested that the optimum feeding frequency of W. pigra was once every three days. Scientific and reasonable feeding amount of B. purificata should be calculated based on oxygen consumption and suffocation point of W.pigra and B.purificata at every period. Meanwhile, stocking density, water area and water exchanging frequency should be taken into consideration.

ABSTRACT

BACKGROUND:Sushi suffocation is relatively uncommon, and it is an unignorable cause of sudden death; however, no reports on sushi suffocation have been published. METHODS:A 60-year-old man was referred to our hospital for post resuscitative intensive care. He had choked on sushi and collapsed in the dining room of a mental hospital. A nursing assistant summoned a physician who attempted to extract the sushi. External cardiac massage was initiated after 7 minutes had elapsed and followed by endotracheal intubation. Return of spontaneous circulation was achieved after 7 minutes of resuscitation. A bronchoscopy demonstrated a large amount of shari in the trachea and right bronchus, which was removed with alligator forceps and a wire basket. RESULTS:Neurological recovery was evident on day 2 of admission. He was transferred back to the mental hospital with no neurological complications. CONCLUSION:Emergency physicians should consider sushi suffocation, including its clinical features and management.

ABSTRACT

Background: The word asphyxia is now commonly used to describe a range of conditions for which the lack of oxygen whether it is partial (hypoxia) or complete (anoxia) is considered to be the cause. The cardinal signs of asphyxia are cyanosis, congestion and petechial haemorrhage. Objective: To investigate the causes and manner of asphyxial death. Materials and Methods: It was a retrospective study conducted in the department of Forensic Medicine, Dinajpur Medical College, Dinajpur during July 2004 to June 2006. A total of 96 cases were examined and recorded. These cases were brought to the morgue of Dinajpur Medical College from 13 different police stations and 1 railway GRP station of Dinajpur district. The data were collected from the autopsy reports (preserved third copy) with permission of the concerned autopsy surgeons. Results: 88.55% of asphyxia deaths were due to hanging, 6.25% due to strangulation and 5.20% were due to suffocation. Conclusion: Most of the asphyxial deaths are due to hanging.

ABSTRACT

A 52 year-old man was admitted to our institution with sudden onset of severe chest and back pain. The electrocardiogram showed ST segment depression in leads I, II, aVL, aVF, V3-6. Emergent coronary angiogram was performed, but the catheter did not reach to the coronary ostia, and it only performed false lumen aortogram. Computed tomography showed acute Stanford A aortic dissection. Ultrasound echocardiography also showed aortic regurgitation 3/4 degree. We decided to perform an emergency operation. During anesthesia induction, systemic blood pressure fell below 80 mmHg during systolic period, and pulmonary pressure raised to 60 mmHg. Transesophageal echography showed the movements of dissection flap intermittently obstructed the coronary blood flow and aortic valve annuls. Those flap movements, so called ‘flap suffocation’ was thought to be the cause of cardiac failure. Intra-operative findings of the ascending aorta showed an entry of dissection just above the left coronary ostia, and the entire detachment of intima to aortic wall. We performed ascending aorta replacement with aortic valve resuspension and fixation of coronary ostia. The postoperative course was uneventful, and he was discharged on the 25th postoperative day. For the precise treatment of acute Stanford A aortic dissection with such coronary ischemia, quick diagnosis and operative correction is essential.

ABSTRACT

About 20 years ago, Deakin and Graeff proposed that whereas generalized anxiety disorder is produced by the overactivity of 5-HT excitatory projections from dorsal raphe nucleus to the areas of prefrontal cortex and amygdala which process distal threat, panic attacks are a dysfunction of 5-HT inhibitory projections from dorsal raphe nucleus to the dorsal periaqueductal gray matter, thereby releasing the responses to proximal threat, innate fear or anoxia. Besides, they suggested that the decrease in 5-HT1A neurotransmission in the hippocampus results in learned helplessness and depression. Accordingly, the Deakin Graeff hypothesis provided a unified frame to the widespread use of 5-HT selective reuptake inhibitors in generalized anxiety, panic disorder and depression. Competitor hypotheses implicate panic attacks with the abnormal functioning of locus coeruleus, basolateral amygdala, dorsomedial hypothalamus or an as-yet-unknown suffocation alarm system. Conversely, cognitive psychologists suggest that panic attacks result from the catastrophic (cortical) interpretation of bodily symptoms. In any event, translational models of panic attack are expected to reproduce the main features of clinical panic, namely, the patient's higher sensitivity to both lactate and CO2, the drug specific sensitivity, the lack of stress hormone responses during panic attacks, the higher vulnerability of women and the high comorbidity with agoraphobia, major depression and childhood separation anxiety. Therefore, here we review the main steps in the experimental approach to anxiety disorders which are paving the route towards a translational model of panic attack.

ABSTRACT

A 57-year old female civilian was suffocated by an intruder. The victim (experienced severe dyspnoea) but violently resisted the assault. Two hours after this event, on admission to a Trauma Centre, she demonstrated left facial swelling with low percutaneous oxygen saturation. Chest X-ray and computed tomography demonstrated pulmonary oedema. This improved dramatically within a short time and she was discharged on the 5th hospital day. Pulmonary oedema induced by suffocation has been reported only rarely. The possible mechan-isms by which pulmonary oedema might form after the relief of airway obstruction are discussed.


Una ciudadana de 57 años fue sofocada por un intruso. La víctima experimentó una disnea severa, pero resistió violentamente el ataque. Dos horas después de este acontecimiento, al ser ingresada en un Centro de Traumas, se le observó hinchazón en el lado izquierdo de la cara, con una baja saturación percutánea de oxígeno. La radiografía del pecho y la tomografía computarizada mostraron un edema pulmonar. Este cuadro clínico mejoró dramáticamente en corto tiempo, y fue dada de alta al quinto día de su hospitalización. El edema pulmonar inducido por sofocación ha sido raramente reportado. Se discuten los posibles mecanismos por los cuales el edema pulmonar podría formarse después de desbloquearse la obstrucción de la vía respiratoria.
